Below you will find our Medical Release Form that each individual must have filled out and turned in before going on any Student Ministry trip or event. The purpose of the form is to have important information, as well as important contacts, available in case of an emergency. The form that is turned in will be good for only one calendar year (so if you turn in a form in October, you’ll need to fill out a new one beginning in January). Once the form is filled out and turned in, it is up to the individual to contact the church office to make any corrections on the form. You may pick up a copy of the form in the Student Ministry Information Rack outside the church offices, or you may print this form at home and bring it in.
